Functional Capacity Assessment (FCA) & Report - approx. 10-12 hours

Mid-cost Assistive Technology (AT) Report - approx. 10–15 hours

Minor Home Modifications (MHM) Report - approx. 10–15 hours

Home and Living Report (SIL / SDA) - approx. 15 hours

Travel costs are charged separately in accordance with NDIS guidelines.

  • Services outside work hours (Mon-Fri 5pm-9:30pm, Sat 9am-5pm) – $220/hour; approx. 2-2.5 hours

Telehealth

  • Services within work hours (Mon-Fri 9am-5pm, Sat 9am-1pm) – $160/hour; approx. 2-2.5 hours

  • Services outside work hours (Mon-Fri 5pm-9:30pm, Sat 1pm-5pm) – $180/hour; approx. 2-2.5 hours

Equipment Trial

  • $200/hour; approx. 1.5 hours

    (Only weekdays 9:00am-5pm; Subject to equipment stores’ opening hours)
